Need Help W.H Tax Adjustment to reduce salary tax

while e-filing refund application, i was confused about the details required to be provided in annex-e like date of tax deducted on salary, mobile phone, ptcl etc. how can we specify a date when deductions are made throughout the year? secondly, mode of payment.... when it is deducted at source, what could be the mode of payment? why not return of i.tax (annex-b) is treated as details of tax deducted/ paid, why this annex-e for refund? how to fill out the annex-e, please explain.
