Can anybody provide me the Income tax with-held for salary form?

I just want to submit the payroll tax and for that i need a copy of with-held form....I checked it on FBR site its not in the xls or pdf format.

1 more thing...kia isse form ke 2 copies submit kerwana hote hein with PO in favor of SBP?

agar aik resident company ka employee kisi foreign country mein transfer ho poora tax year Pakistan na aya ho to uski salary pey b tax ded ho k usey miley gi?
